Receiving the formal letter from American Joint Commission International (JCI) on June 1, 2015, in which announced that Mary’s Orthopedic Hospital, Beijing has passed through the JCI accreditation. The JCI accreditation, patient-centered, is known as the most stringent accreditation. It is the only global professional accreditation organization of hospital healthcare and management and stands for the international health care quality standards for patient care and organization management. It also is the basic condition for the international commercial insurance institutions to pay medical premiums. Passing through the accreditation means that Mary’s Orthopedic Hospital, Beijing has researched the international management standards and become the first national orthopedic hospital got through JCI accreditation, which will certainly encourage all staff of hospital to work with JCI soul, perform the improvement continuously with higher standard and provide safer and high quality medical services to patients.

On May 4, 2015 Chicago headquarters of JCI sent three surveyors here to evaluate Mary’s Orthopedic Hospital, Beijing against JCI Accreditation Standards for Hospital. The surveyors spent four days on-site, making inquiries of the staff and patients, to implement comprehensive check and evaluation on the detailed content of under the hospital’s organization structure (such as, management system, medical care, administration, logistics, fire protection, medicine, information system, hospital infection monitoring, quality index, staff qualifications, device security, medical education, etc.) with varieties of ways, including documents review, depth interview, system tracking and patient tracking, etc. During the four days, the highly compact survey always was safety& quality-centric. At 16:00 on July, all staff of the hospital sitting in the conference hall and anxiously waiting for the result report, the three surveyors came into the hall and sat down with smile on their face. Robert Christmas, the survey team leader, read out the survey result, presented the report to Mr. Lin (president of the hospital) and said: “The report will not let you down.”, and they would submit the survey result to JCI headquarters for final decision.

The hard-won report is full of all staff’s year's efforts with all days and all nights, of which process is more than just words, but now all we did is worthwhile. We’re not alone, because I all hospitals that passed through JCI accreditation feel the same. Domestic and overseas hospitals longing for JCI Gold Medal are countless; however, most of them gave it up when know about the difficulty of the accreditation. So, peers make fun of the JCI accreditation as “Saying love to you is too hard”

Face the Difficulty; True to Form

JCI concept was introduced in the run-up to Mary’s Orthopedic Hospital, Beijing. Through sending staffs out to study and inviting relevant persons to hospital to give guidance, we got a certain understanding of hospital overall safety design and service concept. Our original intention was just for the actual quality of medical, rather than the medal, but during the process of trying to research the JCI standards, we found that our progress is very slow. Then, a year ago, the hospital management committee adopted the decision on the application for JCI accreditation unanimously. The first step was base-line investigation, which promoted the real communication among the hospital staffs and through which we knew that about one thousand hospital hardware facilities needed to be upgraded. The total cost of upgrading, related staff training, software, time, energy and so on were estimated at close to twenty million. Faced with the huge sums of investment, the board of directors made no bones about it, which also encouraged the staffs to try their best to face the difficulty.

As the first orthopedic hospital to be surveyed, we have no experience to learn from. The only thing we can do is to learn, understand and continuously innovate by ourselves, especially faced with the most demanding version, the fifth edition of JCI Accreditation Standards, which includes 368 standards and 1262 measurable elements. Grasping the patient-centered connotation (safety first, quality first) of JCI standards, we gradually carried out the actions on patient assessment, pre-arranged planning, service and tracking and studied about how to promote continuous improvement. During the three hundred days and nights of hard work, we really understood the so called bitter sweet. In the four days of rigorous and meticulous survey, we accepted the baptism of international management standards and point to point operation instruction, deepening our understanding of JCI standards. Only master the in-depth connotation, integrates the content and pay the hardship and effort for it, can we get the essence of JCI standards.

Renew Ideas & Implement JCI Standards

The implementing of JCI standards will inevitably increase much workload, because there are more procedures and processes than before and many things that could not be done before must be done well now. Some confusion occurred to our staff out of incomprehension and some medical staff even was resistant to it during the implementation process. Originally our hospital needed to face with inspections from many national sections. Now, we have to face with the difficult JCI standards, which seems like making a rod for our own back and do not increase any benefit. At the initial training stage, we were confused about the different explanations from different training consultants. All in all, it was so hard to understand and implement the JCI standards accurately before. Through the tough period, the JCI standards turn into a virtuous working circle. JCI not only brings about the form of international development (like, complete hardware and software facilities, clear post duties, institutionalized management, standardized procedures and operations, etc.) to the hospital, but also makes the staff’s get service concept get changed. All the staffs know that they are supposed to really practice the concept of patient& quality-centered and implement improvement continuously.

With the change in the concept and renewal of the idea, the outpatient service process become clear; doctor-patient communication get smooth; work efficiency and patient satisfaction improves. Since the implementation of JCI standards, medical staffs have tasted the sweetness of the normative diagnosis and treatment and the medical team also acquires a completely new outlook; especially for the doctor team, the biggest change is that electronic medical record replaces the traditional paper medical record, which is convenient for all medical staffs and the patients to look up. The application for JCI Accreditation is a tough choice rather than once for all. In the JCI survey articles, it puts that JCI reserves the right from time to time to determine standards compliance. Any actions in contravention of the spirit of JCI will be halted and even taken off the JCI Gold Medal. Certainly, the triennial review is inevitable. Only getting through the review, can we keep the medal. General Manager, Chen Zhenglong introduced that there were many cases that failed in the review and taken off medal in Taiwan. Even in American, there were hospitals that had been taken off the JCI gold medal out of the decline in the quality. In Asia, there are some hospitals voluntarily gave up the second review.

Getting through JCI accreditation is the first step in our long journey and means that we only have stepped into the holy threshold. If don't stick to improving ourselves continuously, will go back to the origin doubtlessly. We firmly believe that this is just a new beginning and it is just like the Incantation of the Golden Hoop, which for us is not only a kind pressure, but a motivation. Let all the staffs take this opportunity work united to seek the higher and further development, sticking to the concept of patient-centered.
